29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut to give maximum protection possible for the operation being performed.  In the lumberyard, On or about June 19, 2014, and at times prior thereto, employees were exposed to the hazard of amputations, when using a DeWalt radial arm saw serial #96756-95, which had an inoperable guard to the lower portion of the saw blade, while cutting lumber.

29 CFR 1910.213(h)(3): Radial saw(s) were not provided with an adjustable stop to prevent the forward travel of the blade beyond the position necessary to complete the cut in repetitive operations:  On or about June 19, 2014, employees were exposed to amputation hazards when using a DeWalt radial arm saw serial #96756-95, which went passed the end of the table, while cutting lumber.

On or about June 19, 2014, employees were exposed to the hazard of amputations when using a DeWalt radial arm saw serial # 96756-95, which did not retract to start position, while cutting lumber.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein.   In the lumberyard, On or about June 19, 2014, and at times prior thereto, employees were exposed to the hazard of burns, as there were no labels, signs as to the contents on or near the 500-gallon container of diesel fuel, while employees refuel the forklifts.
