29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: Bank building exterior: on or about June 19, 2014, employees weather proofing a two story bank building were working from an extensible boom aerial lift. They were not wearing any fall protection exposing them to the hazard of falling from the lift platform.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Bank building exterior: on or about June 19, 2014, two employees were working in an aerial lift installing weather proofing, a third employee was on the ground below them. The third employee was not wearing a hard hat exposing him to struck by hazards from falling objects.
