29 CFR 1926.1101(f)(2)(i): The employer with a work operation covered by the asbestos standard did not ensure that a "competent person" conducted an exposure assessment immediately before or at the initiation of the operation to ascertain expected exposures during that operation or workplace:     (a) On the first floor of the building located at 700 W. Michigan Avenue, Milwaukee, WI, during May and June, 2014, there was no "competent person" provided for this demolition job to identify existing asbestos hazards in the workplace, select appropriate control strategies for asbestos exposure hazards, and having the authority to eliminate those hazards.       (b) On the first floor of the building located at 700 W. Michigan Avenue, Milwaukee, WI, during May and June, 2014, no exposure assessment was performed either before, at the start of the job, or at any point during the job to determine the expected asbestos exposure hazards for workers performing demolition tasks.

Michigan Avenue, Milwaukee, WI, during May and June, 2014, partial demolition was performed by the sub-contractor prior to asbestos remediation, without prompt clean-up and disposal of wastes and debris that tested positive for amosite and/or chrysotile asbestos.
