29 CFR 1926.404(f)(6): Employer did not ensure the path to ground from circuits, equipment, or enclosures was permanent and continuous:  (a)(LOCATION) 238-240 Bedford Ave, Brooklyn, NY, street level, closest to Porta Lavs.   Employees working with wet freshly poured concrete in the basement were exposed to electrical shock hazards because the 100 foot yellow and black in color flexible extension cord energized from the temporary wiring circuit box and draped over the railing into the basement below, did not have a continuous path to ground because the grounding pin at the outlet had been broken off/removed.   NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
