29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Publications information office: Two power strips were connected in series to provide 120VAC electrical service at the work station.  The USDA Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 957347, citation number 1, item number 2, and was affirmed as a final order on March 21, 2014, with respect to a workplace located at 10845 Rancho Bernardo Rd., Suite 200, San Diego, CA 92127.    The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 911911, citation number 1, item number 10, and was affirmed as a final order on August 6, 2013, with respect to a workplace located at 41374 North Shore Drive, Fawnskin, CA 92333.    The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 642658, citation number 1, item number 1, and was affirmed as a final order on March 13, 2012, with respect to a workplace located at 701 N. Santa Anita, Ave., Arcadia, CA 91006.

The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.304(b)(4)(ii)(B), which was contained in OSHA inspection number 910625, citation number 1, item number 5, and was affirmed as a final order on August 1, 2013,  with respect to a workplace located at 602 S.
