29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a- 87 North Main Street, Medford NJ: Employees, exposed to electrical shock, were actively utilizing extension cords whose outer covering were visibly damaged in several areas. These cords were located and being used throughout the worksite on the day of the worksite inspection. Condition observed on or about 6/30/14. b- 87 North Main Street, Medford NJ: Employees, exposed to electrical shock, were actively utilizing an electric circular saw whose plug was removed and its cord was directly wired into a damaged orange extension cord. Condition observed on or about 6/30/14.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) a- 87 North Main Street, Medford NJ: An employee, not wearing or supplied with any fall protection, was exposed to fall hazards of approximately 16 feet to the ground surface below when performing roof shingle rip, tear, replace, and other related activities from a residential structure's roof with a pitch of approximately 10 on 12. The worksite company employees had not received any fall protection training from the company. Condition observed on or about 06/30/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 06/30/14. River Contractors, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315860544, Citation Number 1, Item Number 4a, and was affirmed as a final order on 12/01/2011, with respect to a workplace located at The Club Apartment Homes, Roof of Building 3500, Avalon Court, Voorhees NJ.
