29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  Work Site:  722 Morris Turnpike, Short Hills, NJ  Three employees were observed working at the roof's edge and were exposed to a 22 foot fall hazard to the ground below as no fall protection measures were in place.     Violation observed on or about 11/11/2014.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) designated by the employer:  a)  Work Site: 722 Morris Turnpike and Corporate Office  The employer did not initiate a program for frequent and regular inspections of the jobsite and equipment. The CSHO observed improper ladder set placement and the lack of fall protection systems for employees working at height of approximately 22 feet above the ground.   Violation occurred on or about 11/11/14.  River Contractors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 315860544, citation number 1, item number 1 and was affirmed as a final order on 12/02/2011, with respect to a workplace located at 3500 Bldg Avlon Court, Voorhees, NJ 08043.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)    Work Site:  Roof located at 722 Morris Turnpike, Short Hills, NJ   Extension ladder was not extended at least three feet above the working surface of the roof to allow safe access on and off the roof.  Violation observed on or about 11/11/14.  River Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 315860544, citation number 1, item number 5a and was affirmed as a final order on 12/01/2011, with respect to a workplace located at 3500 Bldg Avlon Court, Voorhees, NJ 08043.

Violation observed on or about 11/11/14.  River Contractors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 315860544, citation number 1, item number 4c and was affirmed as a final order on 12/01/2011, with respect to a workplace located at 3500 Bldg Avlon Court, Voorhees, NJ.
