29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):  (a) Kitchen: A flexible cord providing 120VAc service to a microwave oven was run through a hole in the wall of the cabinet.  (b) Kitchen: A flexible cord providing 120VAc service to a television was attached to the wall surface.  (c) Library: The flexible cord of a power strip was attached to the wall surface.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  (a) Reception: A fire extinguisher was last visually inspected October, 2013.  (b) Hallway: A fire extinguisher was last visually inspected October, 2013.  The USDA Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 957451, citation number 2, item number 3, and was affirmed as a final order on April 7, 2014, with respect to a workplace located at 1634 Black Canyon Rd., Goose Valley Fire Station, Ramona, CA 92065    The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 910625, citation number 1, item number 2, and was affirmed as a final order on July 29, 2013 with respect to a workplace located at 602 S. Tippecanoe Ave., San Bernardino, CA 92408.

(b) Kitchen: A television was provided 120VAC service by an extension cord plugged into a power strip.  (c) Battalion 23 office: A power strip was used to provide 120VAC electrical service to a refrigerator.  (d) Reception: two power strips were plugged into another power strip to provide 120 VAC to the receptionist desk.   The USDA Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 957451, citation number 2, item number 5, and was affirmed as a final order on April 7, 2014, with respect to a workplace located at 1634 Black Canyon Rd., Goose Valley Fire Station, Ramona, CA 92065    The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 911912, citation number 1, item number 8, and was affirmed as a final order on August 6, 2013, with respect to a workplace located at 28500 Highway 243, Keenwild Station, Mountain Center, CA 92561.
