29 CFR 1910.252(a)(2)(ii): Suitable extinguishing equipment was not maintained in a state of readiness for instant use:  Engine bay: Employees were exposed to burns associated with welding hazards when the designated fire extinguisher was not readily available or maintained in a state of readiness as it was over 30 feet away from the welder, the fire extinguisher had a broken gauge, had not been subjected to a monthly or  annual inspection since October 2011.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Tool room: Employees were exposed to inadvertent contact with live 120VAC conductors remaining after a wall mounted heater was removed, the wires are extend from the wall inside the remaining shell of the heater two feet above the floor level in an unlocked room where employees store items such as the steel rack that was immediately in front of the exposed conductor wires at the time of inspection, the employer noted the fact that the wires were hot in October of 2001 by affixing a red tag on the conductor wires.

(b) Engine bay: Employees were exposed to electrical shock where a conduit entering an electrical box servicing the air compressor was not properly attached to the box to effectively close the opening.
