29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the following instances, ingoing nip points and rotating parts were inadequately or not guarded: a) Vision Sorter Area - Vision Sorters #23, 19, and 14 each had inadequately or not guarded ingoing nip points throughout the equipment; b) Optical Sorter Area - Optical Sorters #10, 8, and 6 each had an unguarded ingoing nip point on each side of the feed chain; c) Optical Sorter Area - Optical Sorters #10, 8, and 6 each had an unguarded 5" rotating adjustment wheel; d) Packing Area - The Fortress Metal Detector had an unguarded ingoing nip point on each side of the infeed roll; and e) Applicator Area - The Applicator Belt had an unguarded ingoing nip point on each side of the outfeed roll.

29 CFR 1910.219(c)(2): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely: In the following instances, horizontal rotating shafts were not enclosed: a) Vision Sorter Area - Vision Sorters #23, 19, and 14 had unguarded rotating shafts throughout the equipment. b) Optical Sorter Area - Optical Sorters #10, 8, and 6 had unguarded rotating shafts throughout the equipment. c) Applicator Belt Area - The applicator belt had an unguarded rotating shaft on each side of the outfeed roll. d) Router Machine Area - Each router machine had an unguarded rotating shaft for the rubber fin wheel.
