29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: Dowel Machine #1 - On June 22, 2016, lockout/tagout procedures were not implemented when an employee was unjamming two pieces of wood slats in the outfeed section when the dowel machine unexpectedly started up causing injuries to the employee's left thumb and index fingers.
