29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a. On or about June 25, 2014 on the south side of a commercial structure, an employee was using a scaffold that did not have suitable means to access the scaffold platforms, exposing employees to a 15 feet fall hazard.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a. On or about June 25, 2014, on south side middle of the structure, two employees welding bar joints to the structure were not protected from a 16 feet 7 inch fall hazard by the use of a personal fall arrest system.
