29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: At this multi-family residential site project, a pump jack scaffold legs were not erected on base plates exposing employees to falls when working from the scaffold.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. At this multi-family residential site, working from a pump jack scaffold platform over 10 feet in height, employees did not have guardrails or other suitable protection from falling to the ground below.
