29 CFR 1910.147(c)(7)(i) as required by 1960.8(b): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   Sleeping Bear Dunes National Lakeshore, Employees exposed to hazardous energy while servicing and/or repairing machines or equipment such as, but not limited to, air compressors, site pedestal, welders, distribution systems and vehicle lifts were not trained on established energy control procedures.

29 CFR 1910.213(d)(1) as required by 1960.8(b): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  Sleeping Bear Dunes National Lakeshore, at the Warner House, a portable Dewalt Table Saw was used to crosscut cedar roofing shingles. The portion of the blade above the table was not guarded there by exposing employees to an amputation hazard.

29 CFR 1910.215(b)(9)as required by 1960.8(b): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:   (a) Empire Maintenance Facility, Mechanics Shop, the Delta abrasive wheel bench grinder, Model # 23-850 was not provided with a tongue guard for the abrasive wheel on the right side of the bench grinder. There by exposing employees to a struck by hazard.

29 CFR 1926.501(b)(1) as required by 1960.8(b): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Sleeping Bear Dunes National Lakeshore, at the Warner House, employee's were installing cedar shingles on a roof near unprotected edge ten feet above the ground without guardrails, safety nets or personal fall arrest systems.  Employees exposed to a fall hazard of up to 14 feet to grass surface.

Employee was exposed to an amputation hazard from an unguarded blade of the table saw.
