29 CFR 1910.147(c)(4)(ii) as required by 1960.8(b): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about November 2, 2016, the employer did not provide sufficient detail and adequate guidance in the existing Hydraulic Vehicle Lift energy control procedures for an authorized employee to clearly understand how to safely and effectively utilize the energy control procedure in that the following areas were not addressed:    1. Types and Magnitude of all forms of energy, its hazards and the methods of controlling the energy.  2. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of energy control measures.  3. How all forms of stored energy in the system is released.  4. How to return the equipment to a normal operating condition.

29 CFR 1910.147(c)(6)(i) as required by 1960.8(b):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   On or about November 2, 2016, the employer did not conduct an annual inspection of the energy control procedures to ensure that they were properly implemented during servicing and/or maintenance on equipment.
