29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard: a) On or about June 27, 2014 and times prior employees were exposed to injury from falling to the lower surface. The caster machine work platform was not adequately guarded exposing the operator to falls while operating the unit. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) On or about June 27, 2014 times prior, employees were exposed to injury while working near the pulling cable used to move the casting unit which was under tension. In the event of failure, the cable could strike a worker and cause death or serious injury. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
