29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) On October 6, 2018 and at times prior, an employee suffered lacerations on his fingers from a scraper inside a 6 feet diameter concrete mixer. The employee was in the process of opening the door of the mixer during the shutdown cycle while the scraper was still moving inside the mixer. There was no energy control procedures specifically outlining the steps for shutting down the concrete mixer. The employee's index finger was later amputated. Abatement Note: The procedures must include a method for employees to positively determine that the mixer blades have come to a complete stop before opening the mixer doors (waiting a pre-determined time from turning off the mixer and opening the doors is not acceptable).
