29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a.)  On or about June 25, 2014, at the above addressed job site, employees were exposed to a fall hazard while preparing and painting surfaces on a roof without using a means of conventional fall protection.  PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED.

On or about June 25, 2014, at the above addressed jobsite, employees who had not been provided training to recognize fall hazards were exposed to a fall hazard while preparing and painting surfaces on a roof without using a means of conventional fall protection. 29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a.)   On or about June 25, 2014, at the above addressed jobsite, an employee was using a half face respirator while performing spray painting.
