29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about March 30, 2015, at the addressed worksite, the employer did not properly and safely install a horizontal lifeline used as the anchor point of a personal fall arrest system for employees performing power washing work on a roof approximately 40 feet in height.
