Instance 1) Employees were exposed to fall hazards greater than 6 feet at an unguarded second floor balcony, located at the top of the stairs. Instance 2) An employee fell greater than 6 feet from the attic stairwell opening to the second floor below, where the floor opening did not have a guardrail. THE COMPANY, ANDERSON INSULATION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313796203, CITATION NUMBER 1, ITEM 3, ISSUED ON DECEMBER 21ST, 2009 WITH A FINAL ORDER DATE OF JANUARY 7TH, 2010.
