29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: a) MIRS Department: Employees were exposed to caught-in hazards, in that; the employer had not implemented effective energy control measures when clearing jams on two COLMEC TGF150/15D extruders. Employees removed guards and bypassed electrical interlocks at the lower portion of the die hopper.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) MIRS Department: Employees were exposed to amputation hazards from ingoing nip points, in that; employees operated a COLMEC TGF150/15D extruder without a guard over the lower portion of the die-hopper.
