29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:     a. On or about June 30, 2014 - at the above addressed jobsite, the employer failed to provide a written certification record of training for each employee exposed to fall hazards.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a.  On or about June 30, 2014 - at the above addressed jobsite, employees performing carpentry operations from the third floor deck, more than twenty five (25) feet above the ground were not protected from falls to the outside of the building, thereby exposing the employees to fall hazards.    b.  On or about June 30, 2014 - at the above addressed jobsite, employees performing carpentry operations from the third floor deck, approximately eleven (11) feet above the lower level were not protected from falls to the inside of the building, thereby exposing the employees to fall hazards.    P&S CARPENTRY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 962370, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 22, 2014, WITH RESPECT TO A WORKPLACE LOCATED AT 3642 BELL AVE., CHICAGO, IL  60618.    P&S CARPENTRY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 901091, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER JULY 3, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 3515 N. OAKLEY AVE., CHICAGO, IL  60613.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
