Note: In addition to abatement certification, the employer is also required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.

(a) Worksite, west side of building - Two employees installed clay tile roofing along the west side of the building at a height of 14'-7" while working from a JLG Articulating Boom Aerial Lift, Model # 400S / Serial # 0300134997, without fall protection on, or about June 27, 2014. (b) Worksite, south side of building - Two employees installed clay tile roofing along the south side of the building at a heights ranging between 10'-6" and 21' while working from a JLG Articulating Boom Aerial Lift, Model # 400S / Serial # 0300134997, without fall protection on, or about June 27, 2014. Note: Abatement for this violation was previously documented; therefore, the employer is not required to submit abatement certification or documentation for this violation in accordance with 29 CFR 1903.19.

29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: (a) Worksite, west and south sides of the building - The maximum restricted basket capacity of 500 lbs. was exceeded by approximately 115 lbs. when the JLG Articulating Boom Aerial Lift, Model # 400S / Serial # 0300134997, was operated with the boom extended within the manufacturer's "restricted capacity" zone, at a distance of approximately 31'-7". The aerial lift's basket was occupied by two employees, clay roofing tiles, fasteners, and miscellaneous hand tools, which had an approximate combined weight of 615 lbs. on, or about June 27, 2014. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.
