29 CFR 1910.30(a)(2): Portable dockboards shall be secured in position, either by being anchored or equipped with devices which will prevent their slipping.   a) Warehouse - On or about July 02, 2014 employees were required to load and unload trailers utilizing an unsecured steel plank. Employees were thereby exposed to the hazards associated with an unsecured walking / working surface.

29 CFR 1910.177(c)(1)(i): The employer shall assure that no employee services any rim wheel unless the employee has been trained and instructed in correct procedures of servicing the type of wheel being serviced, and in the safe operating procedures described in paragraphs (f) and (g) of this section.   a) Tire Services Company - On or about July 02, 2014, employees were required to mount and dismount tires from various types of rims.  The employer did not ensure that each employee had been trained and instructed in correct procedures of servicing the type of wheel being serviced, and in the safe operating procedures described in paragraphs (f) and (g).  Employees were thereby exposed to struck-by hazards.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).   a) Tire Services Company - On or about July 02, 2014, employees were required to operate powered industrial vehicles daily. The employer did not ensure that each operator had successfully completed the training required by paragraph (l), thereby exposing employees to struck-by hazards.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.   a) Tire Services Company - On or about July 02,2014, employees used powered industrial vehicles on a daily basis.  The powered industrial vehicles were not inspected for defects on a daily basis prior to use, thereby exposing employees to struck-by hazards.

If such hazards are present, or likely to be present, the employer shall:   a) Tire Services Company - On or about July 02, 2012, in the facility, employees were required to don and wear personal protective equipment such as gloves, hearing protection, and safety glasses while performing their assigned tasks.  The employer failed to perform a hazard assessment as required by 1910.132 (d)(1).

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.  a) Tire Services Company - On or about July 2, 2014, employees were allowed to use unregulated compressed air (not reduced to less than 30 p.s.i.) to clean, thereby exposing them to the hazards associated with flying chips / debris.
