29 CFR 1910.23(a)(2): Every ladderway floor opening or platform with the passage through the railing was not provided with a swinging gate or so offset that a person cannot walk directly into the opening: On or about July 3, 2014, ladder platform openings installed on elevator legs, grain bins, and in the head house were not equipped with swinging gates and were not offset from the platform.
