The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following:   29 CFR 1910.28(b)(3)(i)(A):  Covers; 29 CFR 1910.28(b)(3)(i)(B): Guardrail systems; 29 CFR 1910.28(b)(3)(i)(C): Travel restraint systems; or 29 CFR 1910.28(b)(3)(i)(D): Personal fall arrest systems.  a. On or about February 19, 2021, the employer failed to prevent  employees from falling through, or tripping in, a hole at the ladder on the ladder landing platform  for the ladder mounted to storage bin 2. Employees have exposure to a fall hazard of approximately 22 feet when accessing the ladder platform to use the ladder for access to the storage bin catwalk.  b. On or about February 19, 2021, the employer failed to prevent employees from falling through, or tripping in, three holes in the bin catwalk that provides inspection access to storage bins #2, #3, #4 and #5. There was a hole on the catwalk at Bin #2, on the catwalk at Bin #3 and on the catwalk to Bin #4. Employees have exposure to a fall hazard of approximately 60 when accessing the storage bin catwalk to perform regular inspections.
