[63 FR 33450, June 18, 1998] a) On or about 7/2/2014, the employer failed to provide suitable facilities for the quick drenching or flushing of the eyes and body where there was a potential for employees to come in contact with corrosive chemicals, such as but not limited to: hydrogen peroxide, acetic acid and fluorosilicic acid, in that the employers eyewash station was disconnected at the t-valve. b) On or about 7/2/2014, the employer failed to provide suitable facilities for the quick drenching or flushing of the eyes and body where there was a potential for employees to come in contact with corrosive chemicals, such as but not limited to: hydrogen peroxide, acetic acid and fluorosilicic acid, in that access to the eyewash station was obstructed by pallets of material and chemical storage.
