29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    a.)    On or about July 2, 2014, the employer did not assure employees utilized hazardous energy control procedures developed for safe performance of service and maintenance. Employees used the emergency stop button to unjam sheets in the B&C Series Multi Roll Ironer, exposing them to amputation and/or caught by hazards.      b.)    On or about July 2, 2014, the employer did not assure employees utilized hazardous energy control procedures developed for safe performance of service and maintenance. Employees used the emergency stop button to repair and replace belts and ribbons on the B&C Series Multi Roll Ironer, exposing them to amputation and/or caught by hazards.      c.)    On or about July 2, 2014, the employer did not assure employees utilized hazardous energy control procedures developed for safe performance of service and maintenance. Employees do not lock out the electrical disconnect when repairing and replacing belts and ribbons on the American Hypro ironer, exposing them to amputation and/or caught by hazards.         Buckeye Launderers and Cleaners, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1910.147(d)(2), which was contained in OSHA inspection number 646739, citation number 1, item number 2c and was affirmed as a final order on February 20, 2013, with respect to a workplace at the 1805 Oakwood Ave. in Napoleon, Ohio.
