29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources including but not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat:  A.) LORD Corporation, 601 South Street, Saegertown, PA 16433; Building 8B, TS-700 process area: Employees utilized flammable solvents including but not limited to Xylene that has a flash point of 81 degrees Fahrenheit to clean product from process vessels.  A hose was used to dispense the flammable solvent into the production vessel through hatch located on top of the vessel. Procedures/practices were not established to ensure the hoses integral ground wires maintained proper electrical continuity into order to prevent static build-up and the associated discharge of static electricity.    B.) LORD Corporation, 601 South Street, Saegertown, PA 16433; Building 8B, TS-700 process area: Employees utilized flammable solvents including but not limited to Xylene that has a flash point of 81 degrees Fahrenheit to clean product from process vessels.  A hose was used to dispense flammable solvent into the production vessel through hatch located on top of the vessel. While dispensing the flammable the flammable solvent, the hoses nozzle was not electrically interconnected with process vessel in order to eliminate the potential for static discharge.
