29 CFR 1910.134(g)(4)(i):During interior structural firefighting, employees who entered Immediately Dangerous to Life and Health atmospheres did not remain in visual or voice contact with one another at all times: a. LORD Corporation, Building 8, on or about 02/09/25, and times prior thereto: employee entered into the east section of building 8 while donning a self contained breathing apparatus during an emergency event that contained toxic smoke, and resulted in an explosion. In addition, visual or voice contact was not established during the interior structural fire fighting event. Four Fire brigade members responded to a "fire/smoke" event that occurred on February 9th, 2025 that subsequently lead to an explosion in mix tank #2 in building 8 of the facility.

LORD Corporation, Building 8, on or about 02/09/25, and times prior thereto: four fire brigade members entered into building 8 while donning self contained breathing apparatus's during an emergency event that contained toxic smoke, and resulted in an explosion. At least two employees were not located outside of the building 8 area to provided potential rescue or assistance. Four Fire brigade members responded to a "fire/smoke" event that occurred on February 9th, 2025 that subsequently lead to an explosion in mix tank #2 in building 8 of the facility.

29 CFR 1910.156(c)(4): Written procedures that describe the actions to be taken in situations involving special hazards were not included in the training and education program: a.) LORD Corporation, Building 8, on or about 02/09/25, and times prior thereto: Written procedures were not developed to describe the actions to be taken by the fire brigade members in the event of special hazards including, but not limited to, fire events that could lead to explosions in mix tanks and other equipment onsite. Four Fire brigade members responded to a "fire/smoke" event that occurred on February 9th, 2025 that subsequently lead to an explosion in mix tank #2 in building 8 of the facility. b.) LORD Corporation, Building 8, on or about 02/09/25, and times prior thereto: Written procedures were not developed to describe the actions to be taken by the fire brigade members in the event of special hazards including, but not limited to, fire events that involve water reactive materials/chemicals. The job site included water reactive materials/chemicals including, but not limited to Triethoxyvinylsilane. c.) LORD Corporation, Building 8, on or about 02/09/25, and times prior thereto: Written procedures were not developed to describe the actions to be taken by the fire brigade members in the event of special hazards including, but not limited to, how the fire brigade would coordinate with outside fire departments, and local agencies, in the event of fire or other emergency and establishment of the incident command structure.

The junction box and associated conduit were not rated for class I division 2 locations.
