29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Jobsite, 152 Roseberry St. - An employee was performing stucco operations from one plank on a tubular welded frame scaffold.  The employer did not ensure that scaffold was fully planked on the working level, exposing the employee to a fall hazard of approximately eighteen (18) feet.  Observed on or about 6/3/14.  b) Jobsite, 152 Roseberry St. - Employees were performing stucco operations from one plank bridged between two tubular welded frame scaffold sections.  The employer did not ensure the working level was fully planked, exposing the employees to a fall hazard of approximately eighteen (18) feet.  Observed on or about 6/3/14.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:     a) Jobsite, 152 Roseberry St. -  A ladder or other safe means of access was not provided to employees performing stucco operations on a tubular welded frame scaffold.  Employees climbed down the frames of the scaffold and were exposed to fall hazards up to eighteen (18) feet above ground level.  Observed on or about 6/3/14.
