(b) In the second warehouse area, Southwest corner of aisle #21: while loading and unloading merchandise from a damaged steel storage rack. (c) In the second warehouse area, Northeast corner of aisle #21: while loading and unloading merchandise from a damaged steel storage rack. (d) In the second warehouse area, Northeast corner of aisle #22: while loading and unloading merchandise from a damaged steel storage rack.

(b) In the forklift charging area, in the main warehouse Northwest corner: while connecting forklifts to the charging station that had 208V/240V/480V power supplied from a blocked electrical panel.
