29 CFR 1926.954(f): Ground(s) was/were not placed between the work location and all sources of energy:   (a) Jobsite - On or about July 9, 2014 a line crew was installing new power distribution lines and transformer at dead-end pole.  A protective ground was not placed on the power line close to the location where a lineman was connecting the transformer.
