(a) Pole with Tag 336-04-34 -On or about December 17, 2019 the employer did not ensure employees followed the procedure to deenergize the lines, exposing employees to electrical shocks.

(a) Pole with Tag 336-04-34 - On or about December 17, 2019 the employer did not ensure employees had tested the line to ensure it was deenergized prior to starting work, exposing employees to electrical shocks.
