29 CFR 1926.451(d)(3): The inboard ends of suspension scaffold outrigger beams were not stabilized by bolts or other direct connections to the floor or roof deck, nor did they have their inboard ends stabilized by counterweights: On our about June 23, 2014, at the above addressed jobsite, two employees were observed operating a suspension scaffold, for which the number of counterweights were less than that required per the manufacturer's requirements and/or to maintain a safety factor of at least 4:1.
