On the Detroit 5 Ton crane Serial # 150916, the attached spreader bar was not inspected, tagged or rated. 2. On the Demag 10 Ton crane Model # 235H20 Serial # 41912539, the attached spreader bar was not inspected, tagged or rated. 3. On the Yale 5 Ton crane Model # BEW5X19BM1554 Serial # 04B04K159, the attached spreader bar was not inspected, tagged or rated.

29 CFR 1910.179(g)(1)(v): Pendant control boxes were not constructed to prevent electrical shock or clearly marked for identification of functions: On or about July 9, 2014, and times prior thereto, employees in the production department operated overhead cranes with pendant control boxes to lift concrete materials. The pendant control boxes had open holes, missing dust covers and had no identification markings, exposing the employees to an electrical shock hazard and being struck-by and crushed-by the load. 1. GAFFEY 10 Ton Crane Serial # 99-2046 pendant buttons were not marked, missing dust covers and had unused openings. 2. Detroit 5 Ton Hoist Serial # 150916 pendant buttons were not marked, missing dust covers and had unused openings. 3. Demag 10 Ton Model 325H20 S/N 41912539 pendant buttons were not marked, and missing dust covers. 4. Yale 5 Ton Model BEW5X19BM1554, Serial # O4B04K159, pendant buttons were not marked and missing dust covers. 5. OMI 10 Ton Serial # 15459, pendant buttons were not marked, and missing dust covers.
