29 CFR 1926.502(f)(1)(i): When mechanical equipment was not being used, the warning line was not erected no less than 6 feet (1.8 m) from the roof edge: a) Jobsite, Corner of 9th Street and Midland Avenue, Midland, PA: On or about July 9, 2014, during roofing work, the employer did not ensure that the warning line used for fall protection was properly erected on all sides of the roof in that the flag line was not erected at least 6 feet from the edge of the roof in all areas, including, but not limited to the side of the building where the entryway was located, where the flag line ran across the entryway over to the main building in such a fashion that the flag line passed beyond the edge of the roofs, creating an open space between the building and the flag line that employees could fall through.
