29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:    a) On or about July 10, 2014-Job Site at 232-09 64 Avenue Oakland Gardens, NY 11364    Employers permitted employees to use an energized power strip designed for indoor, non commercial use, to provide     power to a an operational electric ladder hoist on a metal ladder, that could be energized and that was in use without     GFCI protection.    WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a) On or about July 10, 2014-Job Site at 232-09 64 Avenue Oakland Gardens, NY 11364    Employers permitted employees to use an energized power strip designed for indoor, non commercial use.    b) On or about July 10, 2014-Job Site at 232-09 64 Avenue Oakland Gardens, NY 11364    The Employer added a replacement female plug end to a flexible cord set without maintaining the original insulation    characteristics. The employer used electrical tape to provide added insulation to un-insulated wire.      WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.
