29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about July 15, 2014, and times prior thereto, employees performing roof work using a Hitachi NV 45AB2 1-3/4 inch Coil Nailer were not provided with eye and face protection, exposing employees to eye and face injuries. Ruben Pizano Carillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a)(1), which was contained in OSHA inspection number 312123375, citation number 1, item number 2 and was affirmed as a final order on September 16, 2009, with a final abatement date of August 28, 2009, with respect to a workplace located at 308 Brutas Blvd, Lewisville, TX.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about July 13, 2104, and times prior thereto, employees installing a roof at a residential construction site were exposed to fall hazards of approximately 9 feet to the ground below.
