29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about October 23, 2014, employees operating a Hitachi nail gun, serial number 44501, were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about October 15, 2014, employees working from the residential roof installing roofing material were not provided fall protection or where an employee wore a harness was not connected with a lanyard to an anchor point exposing them to the hazard of falling approximately 18 feet to the ground below.  Ruben Pizano was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 953051, Citation Number 1, Item number 1 and was affirmed as a final order on February 26, 2014, with a final abatement date of December 23, 2014, with respect to a workplace located at 5903 Mira Lago, Arlington, Texas 76017.
