29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). The employer does not use non-supporting ladders at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter the working length of the ladder. a. This violation was observed on or about July 14, 2014, on the NE corner of a single family dwelling where an employee was exposed to an 8 foot fall hazard when accessing a roof without a 28-foot extension ladder being used at an angle of 4 to 1. b. This violation was observed on or about July 14, 2014, on the central west roof of a single family dwelling where an employee was exposed to an 8 foot fall hazard when accessing a roof without a 28-foot extension ladder being used at an angle of 4 to 1. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that non-self-supporting ladders are used at an angle such than the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). The employer does not protect employees from falling when working 6 feet or more above lower levels. This violation was observed on or about July 14, 2014, on the roof of single family dwelling where employees were exposed to an 8 foot fall hazard when performing roofing activities on a residential structure without a fall protection system. Arnulfo Rodriguez Roofing, Co., Inc. dba Rodriguez Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 960351, citation number 1, item number 1 and was affirmed as a final order on March 7, 2014, with respect to a workplace located at 3003 Meadow Prairie St., Katy, TX 77494. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees engaged in residential construction activities 6 feet or more above lower levels are protected from falling.
