Instance a)  This violation was most recently observed on about December 15, 2014 in the 1,3 Butylene Glycol Unit at OXEA Corporation located in Bishop, Texas when the employer did not ensure that the Hydrogen reactor T-221 has a manufacturer nameplate.       Instance b)  This violation was most recently observed on about December 15, 2014 in the 1,3 BG Unit at OXEA Corporation located in Bishop, Texas when the employer did not ensure that the manufacturer nameplate on Hydrogen reactor T-222 is legible.
