29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) Job Site - Employees were not provided with adequate training in hazard recognition as evidenced by an employee applying Karnak roofing cement without wearing eye protection, and an employee using a propane torch to heat roof flashing and fire extinguishing equipment was not available, on or about 04/01/2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Also, the top of the ladder was one foot from the face of the roof landing, on or about 04/01/2014.   Abatement certification required within 10 days after abatement date. National Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 753041, citation number 1, item number 1, and was affirmed as a final order on 03/13/2013, with respect to a workplace located at 5050 MacArthur Blvd, NW, Washington DC 20036.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

National Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 753041, citation number 1, item number 2, and was affirmed as a final order on 03/18/2013, with respect to a workplace located at 5050 MacArthur Blvd, NW Washington DC.     Abatement certification and documentation required within 10 days after abatement date.
