On the 4th floor Area C-1, the 4-A-60 BC portable fire extinguisher used in the event of a fire had been discharged and was empty thereby exposing employees to a fire hazard. b. On the 2nd floor Unit 232, the 4-A-60 BC portable fire extinguisher used in the event of a fire had been discharged and was empty thereby exposing employees to a fire hazard.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. At the entrance to the roof area of A-4/C-1 where employees were accessing to connect the air conditioner disconnects, the break in elevation was 33 1/2 inches thereby exposing employees to a fall hazard.
