29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and with no obstructions: Worksite: On or about July 16, 2014, and at times prior thereto, two employees working in the surge bin area, crossing between the hoppers where anchor bolts were sticking out of the floor. This condition exposed the employees to trip hazards.

Next to and behind the hoppers, there were 2 gray 25 foot 12 AWG homemade extension cords connected together that were not approved. This exposed employees to electrical shock hazards.
