29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 11 HEARTHSTONE DRIVE, READING PA, ROOFTOP - Two (2) employees performed roofing work, and the employees were not protected by any means of fall protection, thereby exposing the employees to fall hazards ranging from approximately fifteen (15) to twenty (20) feet to the ground below, on or about July 11, 2014. (b) 13 HEARTHSTONE DRIVE, READING PA, ROOFTOP - Two (2) employees performed roofing work, and the employees were not protected by any means of fall protection, thereby exposing the employees to fall hazards ranging from approximately fifteen (15) to twenty-two (22) feet to the ground below, on or about July 11, 2014. No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 11 HEARTHSTONE DRIVE, READING PA - Employees accessed the roof of the residence using a Louisville twenty-four (24) foot extension ladder, and the rungs of the ladder only extended approximately thirty (30) inches past the upper landing surface, thereby exposing employees to serious injury, on or about July 11, 2014. (b) 13 HEARTHSTONE DRIVE, READING PA - Employees accessed the roof of the residence using a Louisville thirty-two (32) foot extension ladder, and the rungs of the ladder only extended approximately eighteen (18) inches past the upper landing surface, thereby exposing employees to serious injury, on or about July 11, 2014. No abatement certification or documentation required.
