29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about 7/18/14, two employees were not provided safe means of egress from a trench with an approximate depth of 14 feet. The employees utilized an extension ladder to reach the top of the 10 foot tall trench box and then traversed to the top of the 14 foot deep trench, exposing them to fall hazards.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: On or about 7/18/14, an employee was observed standing on the spreader bar of a 10 foot tall trench shield without any form of fall protection.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): On or about 7/18/14, two employees were exposed to cave in hazards while removing the steering head from a core bore in a trench with an approximate depth of 14 feet. The slope on the north end of the trench was approximately 42 degrees while the requirement was 34 degrees. Additionally, the shield system was placed approximately 4 feet below the top of the vertical side of the trench on the north east corner, failing to meet the requirement of 18 inches or more above the trench.
