29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. The employer does not ensure that energy control procedures clearly and specifically outlined the steps for shutting down, isolating, blocking and securing machines. This violation was observed on or about July 21, 2014, in the machine shop area where an employee was exposed to struck-by, caught in/or between hazards when cleaning a machine without machine specific procedures that outlined the steps for shutting down and isolating the machine from its energy sources. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is compliance with the standard, including describing the steps to ensure that energy control procedures clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees The employer does not ensure that a lockout or tagout device is affixed to each energy isolating device by an authorized employees. This violation was observed on or about July 21, 2014, in the machine shop, where an employee was exposed to a struck-by or caught in/or between hazard when performing lock out activities without affixing lockout or tagout devices to each energy isolating device. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is compliance with the standard including describing the steps where employees apply lockout or tagout devices to each energy isolating devices.
