Also, a patch of pachysandra was located on the ground next to the cage.

Punching station.  2.      Shear discharge area.  3.      Angle discharge area.    c)  On, or about, July 22, 2014, three spaces were left in the barrier guard provided on the Wysong shear that allowed entry to the hold-downs and shear blade.    U.S. Chutes Corporation was previously cited for a violation of this occupational safety and health standard which was contained in OSHA Inspection Number 311763908, Citation Number 1, Item Number 9 and was affirmed as a final order on November 30, 2009, with respect to a workplace located at 607 Bantam Road, Bantam, Connecticut.
