Employees using impact hammers and or other specialty tools to tighten field splices (bolts) were not using hearing protection and were potentially exposed to permanent hearing loss.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition: On and before 07/22/2014, the hard hat worn by an employee tasked with completing structural steel erection was cracked and discovered in unreliable condition. The lanyard also used by the employee was damaged and frayed and its deteriorating condition potentially could have exposed the employee to a fall hazard.
